Hospice Criteria
 
  • Has a terminal disease process has been identified?
  • Has the physician given a prognosis of six months or less?
  • Is the focus of care comfort, instead of aggressive or curative
    intervention?

Through our convenient referral process, one call will connect you to our Home Health & Hospice Intake staff. We will coordinate all services including pre-certification for insurance, home medical equipment, medical supplies, IV supplies, etc. Most cases are opened within 24 hours but we can open within hours if requested.

We accept Medicare, Medicaid & Private Insurance.
